Nursing Home Visitation Services
Are you concerned about your loved one in a nursing home, independent or assisted living residence? I will help you communicate your needs and issues to your team of caregivers, loved ones, and nursing home staff. I offer well-being visits devoted to check:
- Satisfaction with services
- Care and maintenance of the living space and personal items
- If personal care assistance is being provided as expected and identifying any needs that are not being met
Advocacy Support Services
Are you struggling to manage your healthcare, or that of a loved one? You don't have to do it alone. I offer consultations to listen, inform you of resources, and ensure you are receiving the highest level of care.
Appointment Support Services
Does your loved one need help at medical appointments? My services help loved ones understand and manage their health journey. During medical appointments and procedures, I will:
- Present your full medical history as well as an integrated understanding of ongoing health goals and needs
- Establish a close working relationship between medical providers to help reduce errors. Open communication minimizes the chances of miscommunication or incomplete information transfer
- Keep your team of caregivers, loved ones, and nursing home staff updated regarding recommendations communicated during the appointment

Initial 30 minute call is free! This is used to assess how I can help you or your loved one. An assessment and service plan is required for clients who purchase visitation, advocacy or appointment accompaniment services. All paid services are provided on a retainer-fee basis.
